I am a 48-year-old male of Native American heritage with a Ph.D. in Humanities, which means I fall within a category of extremely underrepresented minorities in academia with a Ph.D. I am also a citizen in two different nations on the North American continent: the United States of America and the Muscogee/Creek Nation, so being in unique/unorthodox categories is nothing new to my life.

However, six months ago I found myself forced into a new minority group: men that have been sexually harassed — grabbed or pulled or touched without permission — by their female administrators (which has led to retaliation against me for filing a sexual harassment claim and the subsequent denial of my tenure — more about all this later).

This new minority group that I now find myself to be a part of is not a group that I chose to represent (quite frankly, it has taken me months of psyching myself up to have the courage to write this embarrassing article).

I hope that the following narrative of my experience will help others (men and women alike) to speak up and out against people in positions of power that are abusing their power and abusing people in the process; and I hope that it becomes clear that the paradigm of men with power over women has become reflected in the actions of women with power over men and I hope that #me(n)too! is a concern that will now be addressed constructively.

What Happened

On Aug. 17, 2017 most Truckee Meadows Community College (TMCC) employees were gathered for a Semester Kick-off Event, led by TMCC’s President, Karin Hilgersom, and her administrative staff.

At 1:47:59 into this event, extremely loud music began to blare throughout the room, drowning out the speaking President.

It seemed like the booming audio was a mistake, but when it was not immediately silenced, and individuals around the room began to stand on their chairs, shouting, clapping, and swaying, it became apparent that the shrieking music was not a mistake.

Within a minute, the individuals standing on their chairs, shouting, clapping, and swaying, jumped to the ground and made their way to the front of the room, where they were joined by even more people who started a choreographed dance routine to the music, accompanied by the President and her administrative staff.

I would like to reiterate that this meeting was a mandatory, professional gathering, which broke into a flash mob with people swaying and shaking their bodies in the faces of seated individuals that did not ask to be a part of such an activity.

Since May 1, 2007 JSC “Aeroflot –Russian Airlines” introduces an improved service class “Kommersant” on the flights Moscow – Helsinki – Moscow, Moscow – Geneva – Moscow, and Moscow – Nice – Moscow. Detached area in the forepart of the economy class of the economy class is offered for the passengers of the “Kommersant” class on the aircrafts А320 exploited by Aeroflot on European and domestic Russian routes.

Improved food and service, a range of free alcoholic drinks, extended press mix, additional comfort things will be offered depending on flights duration. Other significant benefits for passengers of the “Kommersant” are attractive airport services check-in at the business class desk, priority baggage delivery, in-cabin baggage allowance of 15 kg and one personal thing (similar to business class), and also a pleasant compliment will be a voucher to feed zones in the Sheremetyevo airport. Aeroflot Bonus participants will get 150% miles for such a flight. Tickets of this class can be returned at any time before the flight with 100% reimbursement.

The “Kommersant” class is unique for Russian airline companies. It is especially convenient for corporate clients, passengers and businessmen who fly often. In future an improved economy class will be introduced on priority European destinations and other medium haul routes.

Aeroflot is gradually realizing programs of service enhancement and ensuring maximum comfort for passengers. New service classes are being introduced on Aeroflot flights: “President” – luxury business class for passengers on long-haul flights by Boeing 767; “Premier” – an improved business class on Airbuses - А319, А320, А321 for passengers traveling to priority European destinations. Aeroflot activities program on services improvement in 2007 stipulates improvement of the system of product quality control and achievement of the determined level of standards of all service elements.

Abstract

We highlight primary care innovations gathered from high-functioning primary care practices, innovations we believe can facilitate joy in practice and mitigate physician burnout. To do so, we made site visits to 23 high-performing primary care practices and focused on how these practices distribute functions among the team, use technology to their advantage, improve outcomes with data, and make the job of primary care feasible and enjoyable as a life’s vocation. Innovations identified include (1) proactive planned care, with previsit planning and previsit laboratory tests; (2) sharing clinical care among a team, with expanded rooming protocols, standing orders, and panel management; (3) sharing clerical tasks with collaborative documentation (scribing), nonphysician order entry, and streamlined prescription management; (4) improving communication by verbal messaging and in-box management; and (5) improving team functioning through co-location, team meetings, and work flow mapping. Our observations suggest that a shift from a physician-centric model of work distribution and responsibility to a shared-care model, with a higher level of clinical support staff per physician and frequent forums for communication, can result in high-functioning teams, improved professional satisfaction, and greater joy in practice.

By all reports, primary care physicians are at high risk of burnout. 1 3 Fewer physicians are choosing primary care; many are leaving it. 4 6 Although waning interest in adult primary care careers is multifactorial, driven by such forces as the primary care–subspecialty income gap, medical schools’ devaluing of primary care, and the unsustainable primary care work life, we focus on the work life issue. One study suggests that the difficult work life may be the most influential factor discouraging medical students from primary care careers. 7

Those who practice adult primary care are often deeply dissatisfied, 1 spending much of their days performing functions that do not require their professional training. 8 More than one-half of general internists and family physicians have symptoms of burnout. 1 Time pressure, chaotic work environments, increasing administrative and regulatory demands, an expanding knowledge base, fragmentation of care delivery, and greater expectations placed on primary care contribute to the strain. 9 Workdays are getting longer 10 and rewards are diminishing. Joy is in short supply.
